Provider Participation Agreement

Plain-English summary: To be listed, you must be a real, appropriately licensed provider. Keep your listing truthful and current, don't make misleading claims, treat patients and peers with professionalism, and follow advertising law. Premium placement is labeled and disclosed. We can suspend or remove listings that break these rules. Claiming, creating, or keeping a listing means you accept this agreement.

1. Who may be listed

TMJDirectory lists licensed healthcare professionals and legally practicing providers whose work involves temporomandibular disorders — including dentists and dental specialists, orofacial pain specialists, physicians, physical therapists, chiropractors, and massage therapists or bodyworkers where their practice is legally licensed or authorized in their state. By claiming or creating a listing you represent that:

We may ask for — and you agree to provide on request — your license number, issuing state, expiration date, NPI number (if applicable), business address, and proof of identity or insurance. We may check these against state licensing boards, the NPI Registry, and public business records, at listing time and periodically after. Verification confirms identity, licensure status, and control of the practice's contact information; it is not a certification of clinical competency or quality (see How We Vet). TMJDirectory does not certify or independently determine whether a listed professional is a specialist; titles, credentials, and areas of practice are provided by the listed professional unless otherwise indicated.

4. Content you submit

You keep ownership of the bios, photos, and other content you add to your listing. You grant TMJDirectory a non-exclusive, royalty-free license to host, display, and reproduce that content to operate and promote the directory. You represent that you have the rights to everything you upload — no copyrighted photos you don't own, no content that violates another person's privacy (including patient images without written consent).

7. Suspension and removal

We may suspend or remove a listing, without refund of any then-applicable fees, if:

Where practical we'll notify you and give you a chance to fix the issue first. Serious safety or fraud issues may result in immediate removal. You may wind down voluntarily at any time — claim your listing and request removal, and we'll honor it.

8. Relationship of the parties

You are an independent business. Nothing in this agreement creates an employment, agency, partnership, or referral-fee relationship between you and TMJDirectory. We do not direct visitors’ choices; we display information and let them decide. You are not an employee, agent, representative, or contractor of TMJDirectory.

9. General

This agreement works together with the site Terms of Service and Privacy Policy. We may update it as the directory grows; material changes will be dated at the top of this page, and continued listing after a change constitutes acceptance. This agreement is between you and Stanley Wellness LLC (operating as TMJDirectory) and is governed by the laws of the State of Missouri, without regard to conflict-of-law rules.
